If the Seer fails to reveal someone, that person morphs or mimics and then crosses the Seer, the Seer will see their actual name but the disguised form.

On a further reading of the code, it seems like they don't have to fail to reveal them, they just always see through disguises. And it kind of seems like it was always that way?
