[Task]: Move PlantUML diagrams to the Assets folder

Describe the task

This is housekeeping to reduce clutter:

• The /diagrams folder should be moved under the /assets folder.
• Update the root and diagrams README.md files, if needed.
• Ensure that diagrams can still be generated properly.
