Unconnected document nodes in visualizations

If a document has not been published, it still appears as a node for an entity in the D3 visualizations:

This seems to be due to our code not checking document status when rendering the data for the visualization:

Hi all, Sarah completed this document (KYR-0001-017-0131), but that did not resolve our issue with James F. Bond's unattached node

I think this means we would need you both to take a look at fixing this for us. I'll send an email to follow up--I need to see if we need to get your a new contractor form since its technically a new fiscal year. We shouldn't, but, I've been wrong before. I'll follow up soon--Chuck
