Assessment forms all say "Claim for Admin"

Now that we're entering the assessment forms in plain text, the "Claim for [Organization Name]" has been inadvertently changed to "Claim for Admin."
On the Form, we should be able to type something like {{ORGNAME}}, which will be parsed and replaced with the organization's name.
