The subject says "Subject: subject" and there is no zip+4 field for some reason.

There is no +4 field for any of the reps, not just for one above.

I only render the fields that come down in the server response for retrieve form elements

zip4 never seems to come down, but I always send a zip4 in the background anyway. I can show the form element but would prefer to have the server say to show it.
