Basically sometimes she will spread the gas all over the ground making it unescapable and resulting into a wipe.

So after tonight's many attempts I have an educated guess as to why this happens.

No matter the video settings of your PC, if you run your character to the opposite side of Felmyst during p2, she will go out of your vision. If your raid doesn't have a single player within range to see Felmyst, (No players are in the map and npc update bugs out?) then this problem can happen. This situation seems to happen the most when players are forced to run south / towards the fire as the generally grouping spot is pretty far from the breath starting points. This is shown in the above video. I have max settings on my PC and Felmyst goes out of vision pretty easily.

I didn't really have a chance to test this during our raid, it's just something I have thought about after a night of wiping :(

Reproduction:
Can test my thoughts by monitoring behavior when you keep Felmyst within your vision and note the differences when you let Felmyst leave your vision. Make sure to keep max settings.
I would do it myself but the only place I could try is in our progression raids.

Again, this is just an educated guess based on my limited Knowledge of the fight / scripting.

@Dorakrab If you've found a way to prevent this please share the wealth.

seems was fixed on next cd, after i record this video
