User: Bug report on intake demographics

Question ID	intake demographics
Steps to reproduce	I'm trying to complete the final confidential demographics
when required fields are answered, I select next, an error/ re-try blank page shows up disabling me from completing my online form
Expected behavior	to finish my online report so I may print a copy in efforts to hand deliver a copy to my landlord as instructed for me to do so
What happened instead	an error blank page popped up asking me to retry
