Manual Bans seem to be added to notes in 4 entries. #51

Description of issue
Whenever an Admin manually bans (through display-job-bans, which is also used for regular bans) someone, the entry for "MANUAL BAN" will be placed in the notes 4 times, instead of 1.
Difference between expected and actual behavior
It occurs to me only one entry is needed.

Manual-ban someone, check their notes.
