claude code integration only captures user-prompt — no claude responses / learnings are captured

Lucid captures both user prompts and outputs from codex and open code.

For claude code it only captures the user prompt, making it Lucid significantly less valuable.

I've added a fix in io41@3fdf9b5 but that commit relies on #8 so I won't open a PR until that one is merged, or a variation of it.
