Operating surface v0 scaffold (Now / Artifacts / Decisions / Actions / Collaborate)

Goal: implement the “operating surface” framing as lightweight, modular markdown sections with an index pulling them together.

Proposed structure:
• /operating/now.md
• /operating/artifacts.md
• /operating/decisions.md
• /operating/actions.md
• /operating/collaborate.md
• /operating/index.md (pulls sections together)

Each section includes example entries so updates are easy.

Once merged, Lumen can pull to /var/www/lumen.

Acceptance criteria
• /operating/index.md renders the five sections (Now, Artifacts, Decisions, Actions, Collaborate)
• Each section has 1–2 example entries
• Links point to the existing repos/posts (Memory Weave, x402-skill, De Principiis)
• Minimal/no new dependencies
