Allow removing students

This is a feature request to implement the removal of students from courses.

While inactive students can usually just be ignored, there is a real world case when the requested feature would be useful: bulk uploading students to a course with an incorrect csv column order would result in many incorrect students being created. In this case the only existing ways to fix the course are to manually drop the students table or to recreate the course.
