[16.0] product _pack import file xls

Hello
we are trying to import product with their product_pack from xls file, but always obtain error

duplicate key value violates unique constraint "product_product_combination_unique" DETAIL: Key (product_tmpl_id, combination_indices)=(5461, ) already exists.

can you help us with the template xls?
