When switching to Add Stdio Server mode in the MCP Manager, the form fields appear without a sub-title or header indicating "Add Stdio Server". Users see input fields but no context about what type of server they're configuring.

Steps to Reproduce
Open MCP Manager (Ctrl+E).
Press 'a' to add a server, select "stdio".
Observe the form area.
Expected Behavior
A header like "Add Stdio Server" should appear above the form fields.
Actual Behavior
Form fields appear with no contextual header. Only the main modal title "MCP Servers" is visible.
