[BUG] [v0.0.7] MCP Manager Add Stdio Server form has no header indicating the form purpose

When switching to Add Stdio Server mode in the MCP Manager, the form fields appear without a sub-title or header indicating "Add Stdio Server". Users see input fields but no context about what type of server they're configuring.

Steps to Reproduce

1. Open MCP Manager (Ctrl+E).
2. Press 'a' to add a server, select "stdio".
3. Observe the form area.

Expected Behavior

A header like "Add Stdio Server" should appear above the form fields.

Actual Behavior

Form fields appear with no contextual header. Only the main modal title "MCP Servers" is visible.

Bug Confirmed: The render_add_stdio_server function (line 310) sets the form title to " Add MCP Server " without the "- Stdio" suffix that appears in all other forms:

• HTTP form: " Add MCP Server - HTTP " (line 425)
• Registry form: " Add MCP Server - Registry " (line 517)
• Choose Source: " Add MCP Server - Choose Source " (line 182)
• Choose Transport: " Add MCP Server - Choose Transport " (line 245)

User Impact: When users navigate to add a stdio server (Ctrl+E → 'a' → Custom → stdio), they see input fields for Name, Command, and Args without any indication that they're configuring a stdio-type server specifically. This creates confusion and inconsistency in the UI workflow.
