feat: AI proactively manages docs.md and refs/ — users don't touch these

Add protocol guidance for AI to actively maintain design documentation
and reference materials instead of passively waiting for them to exist:

- docs.md: AI creates after each module design alignment, includes
  design intent and rationale for user review
- refs/: AI auto-saves when user mentions mockups, reference code,
  designs, or pastes code snippets as reference
- Both EN and ZH protocol sections updated
