Remove application name from settings

After digging in, it is used to output the package name in the phpDoc blocks of the generated code. Should we keep this and maybe rename it?* @package <?php echo CodeGen::$ApplicationName; ?>

It would help by auto-documenting the generated code. Even if it isn't necessarily the "Application Name" we could use the variable as "Package Name."

I'm fine with that as long as we provide a default. I would imagine that most people don't want to be bothered by this unless we're going to have a config file... or we could think about doing a one-click-install type thing like wordpress does.
