I'd prefer to have the UI in app and send the responses to Airtable via API instead. I'd need the Airtable API key (@ChialiT), but access to the airtable would be better for debugging since i wouldnt have to bug anyone if the responses are hitting and I'd have access to the api reference for the table

UI in app keeps people in the app instead of going away from it. They can stay and give feedback and continue to use the app.
