For the record, the above result is produced because the source mesh is assumed to be covered by the target mesh. It seems that you might have used a FaceWarhouse model as source and 3DMM as a target. However, 3DMM has missing part of had especially back of the head. Thus, I recommend swapping input. It would be work then.
