Release 0.2.1

Important: Remove all Voukoder presets and the existing Voukoder.prm first. The configuration ids have changed. If you don't do that the preset handling might get screwed up, that might force you to reinstall premiere!

Fixes:

• Plugin registration fixed
• Fixed the issue that people see only a vertical line in the output

Unfortunately this was required and missing and caused a run time error in Premiere CS6.
