Remove exports from @svelte-router/core/kernel

Because in MFE scenarios, people might externalize the routing library, it is best to just have a single module name.  As of **v1.04**, everything can be imported from `@svelte-router/core` directly.

v2.0 will remove the sub-module `@svelte-router/core/kernel`.
