Large number of entries causing overflow requires TOC scrolling #66

If the number of entries in TOC is large and goes off screen vertically, the TOC itself doesn't scroll down.

We have a similar problem on a table of contents plugin that preceded this one. My stance is that having that many headings in a single page probably means the page is too long, from an information architecture perspective. In other words, I don't think the plugin should accommodate. That said, I would accept a pull request fixing it, as long as it doesn't add a lot of complication.

For me it's specifically a problem for a page with release notes displaying release notes for 30+ minor releases. The page is too long, but it's technical and if there are 30+ minor releases that's how many there are. Pagination could be an option but it's a static site so it seems like it would be an overkill.
