As demonstrated in this rough (and outdated) mockup:

Should:

Contain document metadata
Contain link to original PDF
Link to section/note titles
Cover full embed on smaller embed sizes
Position as scrollable sidebar overlay on normal embed sizes
Possibly keep on-page sections/notes sticky while scrolling sidebar
