There's a duplication bug which can be performed when dropsmultiplier for mobs is 2x or more.
steps to reproduce:
step 1: give a mob like a zombie an item (example diamond block)
step 2: kill zombie
step 3: item is duplicated (it'll drop 2 diamond blocks)

This is caused by a bug in spigot. I created an issue on paper for it to be fixed.
