Fix Codex engine configuration, recovery, and observability gaps (#65199)
* Fix Codex engine integration correctness gaps
Repair configuration merging and plugin state, preserve engine environment and provider routing, harden retry and subprocess lifecycle handling, and align native Codex accounting and telemetry.

* Address Codex review feedback on telemetry and runtime safety
Normalize provider aliases consistently, apply environment exclusions independently of overrides, keep framed legacy tool payloads out of telemetry, and avoid signaling successfully reaped process groups. Remove the obsolete enclave TOML renderer and cover each fix with regressions.

Finish forced process-group escalation after the CLI closes, reconcile canonical shell filters, and respect all native model option forms. Preserve tool-result provenance in compatibility and OpenTelemetry accounting, with renderer, shutdown, and cross-engine regression coverage. Correct the related reference documentation and ADR.

* fix(codex): retain terminal accounting in unified sessions
Preserve native turn outcomes, source event types, and reasoning tokens when compacting conclusion session payloads. Add sanitized Codex 0.159.3 evidence from run 37096302208, canonical and raw collection regressions, stable compatibility replay, and type-safe documented fields. Keep cache and reasoning subtotals separate from computed totals.
