Moderation and trust levels (moderate but auto-approve users with a number of other approved comments)

I want to review comments from all new users. However, I’d like to auto-approve comments from users with at least a configurable amount of existing verified comments.

This depends on #312 as users need to be authenticated and not just know a trusted user’s email address (sometimes public or guessable information).
