Is there any deadlock potential of this project?

We use this module in our program. Sometimes (a couple of days/weeks), the program will have an additional thread, in the mean time, the program is deadlock (no exceptions). Once I delete that additional thread, the program works smoothly again. But sometimes it still gets stock without additional threads (still no exceptions). We're not sure the root cause of this, does anybody also have this kind of issue?
