Have friendly screen to prompt for username, password, server

If username, password, server are not set or they are but there is an access problem, send user to this screen & don't let them open other screens.

Have a nice work flow so if user installs app then tries to bookmark page, they go to this screen, then they go to add bookmark screen with URL still set.
