Reformat CHANGELOG to CHANGELOG.md

Reformats `CHANGELOG` in Markdown so that it becomes `CHANGELOG.md` and
uses a style identical to the one we've recently established both in
stripe-ruby and stripe-dotnet.

We leave `CHANGELOG` in place for now to redirect people to
`CHANGELOG.md`.
