Bugfixes galore

• Fix replacing wide chars with narrow chars.
• Do not warn about unknown sequences by default.
• Avoid recalculating string display widths when possible.
• Move initial cursor location off screen again during startup.
• Fix processing of split ST sequences across buffers.
• Fix overlaying/inserting combining characters.
