create dialog element

• Make a separate example at awatif-ui/src/dialog/index.html
• No need to create the button element this is replaced by the toolbar element Create toolbar element #440
• The header section should be fixed when scrolling inside of the dialog body
• When the dialog is closed remove the dialog.body to improve performance (to prevent the report from rendering when the dialog is closed)
• The width of the dialog can be shortened from the right side, no need to make the element movable, let's keep it simple
