Blank Duplicate submission syncing a few seconds after each valid submission

This is suddenly posting blank form submissions a few seconds after each successful submission. The valid entry will sync to the Slack channel followed by a completely blank separate submission that is not logged in the forms spreadsheet, but contains a different timestamp than the original

@bcooney85 You've probably already solved this, but just in case you have not yet:

I had the exact same issue in a workspace I was administering several months ago. I reported the issue here on Github, but the fix essentially became removing the script and adding and initializing it again.
