Country/State fields don't work in modal view #193

Describe the bug
The CiviCRM country/state fields don't work when a form is opened in modal view.

To Reproduce

Create a form with the country/state fields (the contact with address template is fine)
Create a page and embed the form using the caldera_form_modal shortcode
View the page and click the button to open the form

Expected behavior
The state and country fields are selectable and state is dependent on country.

Actual behavior
Neither field selectable at all.
