Feature: Implement a group selector to the demo app.

When connecting to a FHIR server to get patient lists, we'll start by selecting a list of Groups to pull patients from.

The current implementation assumes all groups are to used, which makes no sense (eventually) if groups are 'descriptive-only' and have conflicting criteria.
