All invalid requests should, when possible, redirect with error=invalid_request or one of the other defined codes; more detail should go in descriptions. This probably means adding additional columns to https://github.com/microsoft-healthcare-madison/smart-launcher/blob/master/src/config.js#L46 mapping each error into a coarser-grained bucket like invalid_request.
