Cannot create a Azure OpenAI connection. "Failed to create connection"

Hi all,

Im a little stuck getting the prompt flow plugin working in VS Code. I have all the dependencies passing and I'm at the stage of creating a Azure OpenAI connection. I update the api_base and the name fields and either save or click the create connection.
I recieve a error message as follows

Failed to create connection, check output channel for details.

Clicking the view error details button flicks to the output terminal which is empty.

This is a fresh VS Code install.

Any ideas?
