Remove surface column, use first summary name + header row name columns instead

Right now, the surface column is empty for most cases -- it only has a value for the first summary site and for the header row dividing summaries from sites.
In the same conditions, the first summary name are {"YARM-category-totals"} and {"YARM-category-sites"}, respectively.

We should be able to consolidate both into one column containing something like {"YARM-category-totals", surface_name} and {"YARM-category-sites", surface_name}.
