I might just be misunderstanding the script, but possibly there's a bug in osm_to_pq.py. As I understand it, only nodes created by clipping to the bounding box should have {start|end}_node_reference=NaN, but as shown below, many more than that do.
It would be useful to have some flag of which nodes are located on the bbox edge for subsequent operations in the pipeline.
