Add configuration for launching the MTAP gRPC gateway, including as a docker sidecar. This should provide functionality and documentation for

• Running an RTF file through the RTF-to-text pipeline
• Running an RTF file through the default pipeline

using a minimal number of restful HTTP requests.

This will require the following features:

• Configuration and commands for deploying the biomedicus default pipeline as a service.
• Configuration and commands for launching the mtap-gateway for b9.
• Docker image with the b9 gateway.
• Documentation on how to do all of this.
