Struggling to get it to work with Packwerk project

I am working in a project that has many packwerk subprojects and trying to use Neotest as my test runner. Running :Neotest run outputs that there no tests found. By running :Neotest summary I see that all spec files in my project are there. What could be the issue? How could I debug this?

I'm not familiar with packwerk but it's odd that :Neotest summary reports tests. What happens if you run tests from the summary window?
