Update custom url scheme IANA registrations to new format

As per Christian's message on signal referring to #266 :

We diverge from the registration template as defined in https://www.rfc-editor.org/rfc/rfc7595.html#section-7.4 Is there another template that I didn't find or why the different format?

I guess we should update to use the template.
