Rcpp and TMB have some conflicts so that some macro variables need to be redefined. See https://github.com/openpharma/mmrm/pull/182/files/00db3d368e79a35f4cc6653498167ddc43a08353#diff-e4067f488f94eefc33c34823e3c82f22180bbbf90834b0476b264fb062e10733

suggest we create some fix in TMB package so that these macro definition can be solved
