A good handoff requires multiple passes to get it right

First of all, thanks this is great!

Now an issue:

Everytime after the handoff is created, I'm now used to ask

review the handoff, see if something important for the next session is missing and patch it

Sometimes I ask that another time.

But It would be nice to have an already complete handoff instead of having to ask 2-3 times to review.

I think this is also partially the model's adhd tendencies but nevertheless worth mentioning.

use the partially written handoff at /home/dev/projects/development/thoughts/shared/handoffs/boundaryless-os/2026-02-21_18-35_phase4-review-complete-codex-next.yaml as a trace to spawn an agents team https://code.claude.com/docs/en/agent-teams to use cass to retrieve the context of the last 4 sessions in chrononological order to enrich then handoff itself

My bad that I'm doing crazy fat projects with lots of context and again the small ctx window of claude ain't helping but maybe we can come up with something that build handoff directly from something like cass?

I don't think its your bad.
Over the past 2 weeks I was doing medical research for a problem my kid was having that sent him to the hospital.
When using CC3 for programming and development tasks, the current /create-handoff was working well.

But when I was doing the medical research, I was eating context up all over the place and my handoffs seemed to suffer a but from what you are describing. A need to have it checks its own work on the handoff. This also coincides with the new built in memory (which I'v diabled in claude) and sometimes the context % check hook isnt working and it will suddenly auto-compact instead of suggest a handoff. I need to look into this for my own work atm.
