bug: form field id missing or duplicate #397

There are a few issues flagged by Chrome (this is on the project list page)

Form field element should have an id or name attribute (16 instances)
Duplicate form field id in the same form (32 instances)

The form inputs in the projects list would fail a basic accessibility test too. None of them have labels.
