Here is the report on wp:

To give another bit of info, instead of re-opening the featured image dialogue if the page is saved and reloaded it will pass the test.

Yet another thing…

If the procedure is followed to add the alt text, but then before accepting the featured image you click on a different image and then click back to the right one then accept, then it passes the test.

And one more…

Type in the alt text and then delete text in the title field. Set featured image and it passes.
