[Patch] Add DISABLE_FALLBACK and DISABLE_LOAD_OPTIONS Configuration Options

This enhancement patch adds a DISABLE_FALLBACK and a DISABLE_LOAD_OPTIONS option.

DISABLE_FALLBACK removes fall back logic. It currently needs to be hard-coded defined via -D.

DISABLE_LOAD_OPTIONS removes load options in all cases, not just removable drives. (there are some systems/customers that have issues when creating boot items in firmware caused by the load options).
