Add Trip sheet

After clicking on add trip sheet, a form similar to this can be shown , which will add the trip details by the user.

Trip Number: Can be automatic from the server.
Tractor Number: Vehicle registration number, taken from dropdown.
Driver Name: name of the driver
Helper Name: Name of the helper
Address: Station from where the vehicle started. Can be a text box (Optional)
Odometer Reading: Starting odometer reading
Origin : Start place
Destination: End place of the trip.
