A/V offset ineffective at times #1

How to reproduce:

Start offset in middle, BPM 40. 4/4.
no matter how A/V offset is adjusted, audio 1st beat is with the visual 4th beat

I suspect this has to do with the A/V offset, start offset algorithm calculation.

I will write some unit tests for the offset algorithms, that should allow me to fix them

Probably will fix #2 too
