Problem

If you try and add a new alert when you have already connected a Slack account that has been revoked/expired the page won't load and will error out. This means it's impossible to add new alerts (of any kind) until the relevant row from the OrganizationIntegration table has been removed.

Reproduce

  1. Add a new alert using Slack

  2. After it is added, revoke access to your Slack. https://slack.com/intl/en-gb/help/articles/360003125231-Remove-apps-and-customised-integrations-from-your-workspace

  3. Delete the original alert and try add a new one. You should see an error and be unable to do it.
