When trying to toggle off/on switch in app, when accessibility displays, Duress is off and greyed out.
If try to tap Duress in accessibility settings, message pops up "Restricted setting.. For your security, this setting is currently unavailable"

Same no matter what tab, broadcast, wipe or test.

Seems like its impossible to activate app under any circumstances unless I am missing something here?

You have to go to Settings > Apps > Duress, then on the top right corner there's the 3 dot menu. Enable the Restricted settings there.

Your comment helped me figure out how to enable AutoNotification on GrapheneOS; without making this change, the "Device & app notifications" settings were entirely greyed out with the "Restricted setting" error. Thank you!
