For VTK versions < 6.2 only mha images are supported in vtkImageBlend and vtkImageTransform. So all transforms other than identity will be ignored. In some cases it helps to manually change the transform the header in the output file.

For newer versions of VTK the Nifti format is supported that does not have these issues.
