Quantum: Various bug fixes for Azure Quantum

Description

• Remove QIO and ADFT features, since those are no longer supported for Azure Quantum service and have both been deprecated.
• Fix broken dependency on from azure.cli.command_modules.storage._client_factory import blob_data_service_factory and replaced with vendored_sdk for azure_storage_blob
• Removed requirement for entrypoint, since most modern providers no longer require entrypoint
• Added check for duplicate provider ids and fail fast before sending to service
• Updated default shots to 100

About Extension Publish

There is a pipeline to automatically build, upload and publish extension wheels.
Once your pull request is merged into main branch, a new pull request will be created to update src/index.json automatically.
You only need to update the version information in file setup.py and historical information in file HISTORY.rst in your PR but do not modify src/index.json.
